In which cases a medical license requires reissuance

The license to pursue medical and pharmaceutical activities is subject to reissuance in the following cases:

- reorganization of a legal entity in the form of transformation;

- reorganization of legal entities in the form of a merger

-change of the name of the legal entity;

-change of the address of the location of the legal entity;

-change of the place of residence, name, surname and patronymic of an individual entrepreneur;

-changing the details of the identity document of an individual entrepreneur;

-change of addresses of places where a legal entity or an individual entrepreneur carries out a licensed type of activity;

-changing the list of works performed, services provided, constituting the licensed type of activity.
