In the conditions of coronary virus pandemic, doctors of a primary healthcare unit of Tatneft, a public joint-stock company, and Al'metyevsk physicians continue to provide planned and emergency surgical assistance to the population of the region. At the same time, they often face difficulties in diagnosis and choice of treatment tactics in emergency patients affected with viral pneumonia.
Pulmonary artery embolism is one of the most formidable conditions, inferior in frequency of lethal outcomes only to myocardial infarction and stroke. The risk of adverse outcomes increases in patients burdened with viral pneumonia and other related diseases.
Recently, a patient from Bavly, 58 years of age, was brought to the emergency room of the Medical Unit of Tatneft in a grave condition. During an examination, a massive pulmonary artery embolism was detected. The slightest physical activity resulted in a significant increase in shortness of breath. Along with this condition, symptoms of viral pneumonia were also found on the CT scan.
Collegiate decision was made to carry out a less traumatic method - the medicinal dissolution of blood clots in the lung vessels (thrombolysis). However, the doctors' team did not achieve the desired effect.
Surgical treatment was the only way to save the patient. Surgical intervention was performed in conditions of artificial circulation of blood. During the operation, the lumen gaps of the primary vascular trunks of the lungs and lobes were blocked by clots, causing impediment of circulation and oxygen enrichment of the patient's blood. It was possible to extract a massive amount of blood clots from the lungs.
The operating team consisted of surgeons Sibagatullin N.G., Faskhutdinov N.G., an anesthesiologist Yakovleva M.A., a perfusionist Zaripov B.A., nurses Matveeva D.V., Shcherbakova N.A., and Nigmatullina G.G. Their professionalism allowed to handle the task perfectly and made this rescue operation possible.
As a result, the patient's condition improved, and his breathing was restored. Currently, he is preparing for a discharge home.
