A meeting of the Republican Clinical Tuberculosis Dispensary's medical council was held, where the results of the institution's work in 2020 were summed up. The meeting was held with the participation of the First Deputy Minister of Health of Tatarstan A. R. Abashev, the head of the Sanitary Protection Department of the territory of Rospotrebnadzor in the Republic of Tatarstan L. O. Borisova, the epidemiologist of the Hygiene and Epidemiology Center N.F. Zainutdinova, the head of the Medical and Social Rehabilitation Department of the Republican Center for AIDS Prevention and Control in RT A.A. Kamaeva, the head of Federal Government Health Institution Medical-Sanitary Division-16 of the Federal Penitentiary Service of the Russian Federation P.L. Anisimov and other invited guests.
The leading indicators of the anti-tuberculosis service activity in 2020 were presented in the report by the chief physician of the Republican Clinical TB Dispensary A.P. Alekseev. He noted that in the context of the pandemic of the new coronavirus infection COVID-19, it was necessary to reorganize the provision of outpatient care to tuberculosis patients and create field medical and nursing teams. Considering the equipment of the bacteriological laboratory for molecular genetic research, the laboratory began to provide diagnostics for the detection of COVID-19 in the daily volume of 192 people per shift, 7 days a week. Currently, work on the laboratory diagnosis of a new type of coronavirus infection continues, with a cumulative total of 11,400 studies.
In 2020, tuberculosis's territorial incidence decreased by 18.8% compared to 2019 and amounted to 23.4 per 100 thousand of population (2019 – 28.8; 2018 – 29.6 per 100 thousand people). The incidence of tuberculosis in the republic is 30.1% lower than in the Russian Federation (41.2) and 28.5% lower than in the Volga Federal District (40.3). The death rate from tuberculosis in 2020 decreased by 7.4% compared to 2019 and amounted to 2.5 per 100 thousand population (2019 – 2.7; 2018 – 2.9).
By reducing the incidence of HIV+TB co-morbidity by 19.0%, the share of HIV-infected patients among newly diagnosed patients decreased and amounted to 20.0% in the structure of all newly diagnosed patients.
In 2020, at the expense of the Republic of Tatarstan's budget, the Republican Clinical Tuberculosis Dispensary received a mobile fluorography complex for 10.0 million rubles. This will make it possible to cover the population of remote rural settlements with examinations to detect tuberculosis more fully and identify other thoracic cavity pathologies.
The work on attracting young specialists continues successfully. In 2020, 12 doctors were hired to the dispensary and its branches: 6 doctors to the RCTD in Kazan and 6 to the branches of the RCTD accordingly.
In conclusion, A. P. Alekseev stressed that the task of the anti-tuberculosis service for 2021 is to stabilize the level of morbidity and mortality from tuberculosis. Morbidity is aimed at the level of 27.0 and mortality at 2.7 per 100 thousand of population.
In her speech, Deputy Chief Medical Officer N. K. Gazizullina presented an analysis of the effectiveness of treatment of tuberculosis patients, paying particular attention to the problems of treatment of patients with multiple drug resistance of the pathogen. The reasons for the separation of patients from treatment were analyzed. The need to organize social support for tuberculosis patients to prevent separation and joint work with narcologists and medical psychologists was emphasized.
Deputy Chief Physician for Childhood R. H. Fatykhova highlighted the issues of organizing early detection and prevention of tuberculosis among children and adolescents in the Republic of Tatarstan in 2020. The importance of mass immunodiagnostics for early detection of tuberculosis among children was noted, as well as the fact that 32 children out of 33 newly diagnosed patients with tuberculosis were detected prophylactically.
During the medical council, the head of the Sanitary Protection Department of the territory of Rospotrebnadzor for the Republic of Tatarstan, L. O. Borisova, presented also. She stressed the importance of resuming preventive examinations to detect tuberculosis in full and organizing full-fledged work in the foci of tuberculosis.
At the end of the medical council, the First Deputy Minister of Health of the Republic of Tatarstan A. R. Abashev addressed the participants. He noted the significant positive dynamics of the leading indicators on tuberculosis in the Republic of Tatarstan, as well as the relevance to date of all areas of activity of the anti-tuberculosis service. He suggested broader cooperation with non-profit public organizations in the fight against tuberculosis.
