A doctor from Moscow held a masterclass on cryobiopsy of a lung patient from Kazan

More than 150 thoracic surgeons from Russia and different European and Asian countries gathered at the Republican Clinical Hospital for lectures and operations.

The first day of the symposium was devoted to masterclasses. Surgeons from different cities and countries performed operations on patients with chest diseases using the most advanced technologies. 

All masterclasses were broadcasted online, and the symposium participants could watch what was happening and ask their questions to the surgeons.

In the RCH of Tatarstan, two such masterclasses were held at once. One of them was conducted by Ilya Sivokozov, Head of the Endoscopy Department, pulmonologist at the Central Research Institute of Tuberculosis in Moscow. The doctor performed a lung cryobiopsy on Kazan patient.

Previously, an operation under anesthesia was necessary to perform a biopsy. Traditionally, the material was taken with tweezers. Often, the material obtained was insufficient, and the patient was referred for a second biopsy. The advantage of cryobiopsy is that this technique allows obtaining a higher quality material and 8-10 times larger in volume than with a conventional biopsy.

"The material is taken away with cold. Perhaps every person in his childhood at least once welded his tongue to a metal swing. This is because the ice pieces that occur at low temperatures bind our tongue to the metal surface. In cryobiopsy, the same principle works. We simply weld the necessary material around a special cryoprobe and tear off a piece of tissue, which significantly speeds up the procedure itself," said Ilya Sivokozov.

This technique has been greatly developed in Moscow. It is used by both pulmonologists and oncologists, who are very interested in the fastest and most accurate diagnosis of patients with suspected lung cancer.

"The possibilities of this method of research are very promising. Cryobiopsy has already become a standard abroad and is successfully making its way here. We talked about this with the leadership of the RCH, the chief pulmonologist of Tatarstan, and the chief thoracic surgeon. In Kazan, there are people who care, doctors who want to study. I am sure that Tatarstan needs such technologies and will appear in the republic in the nearest future, " Ilya Sivokozov said.

Simultaneously, in the next door OR, the chief pediatric trauma surgeon of RT Ministry of Health, Professor Petr Andreev performed the procedure to correct the thorax deformity of a 15-year-old.

Chest deformity is a congenital disease. It leads to a decrease in the breast volume, a person's heart and lungs are squeezed. The patient suffers from shortness of breath, and it is difficult for him to do any physical activity. Treatment of the disease requires surgical intervention.

There are more than 30 methods for correcting chest deformities. The RCH surgeons in Tatarstan invented one of the least traumatic and more effective methods. For the operation, doctors use their own patented tools and technologies.

"We do 10-15 such operations a year. Personally, I have been doing this for more than ten years and have operated on about 130 patients. In the first stages, we did not use our designs. They also perfectly coped with their functions, but they had their nuances from a cosmetic point of view. That's why we invented and patented our plates," said Petr Andreev.

The Implant company in Kazan manufactures the plates used by the RCH surgeons. They are made of medical titanium, have been certified, and meet all medical and technical requirements.

During the operation, the plate is inserted into the patient's chest for 8-12 months. After a few days, the patient already begins to walk, and in a week, he can be discharged home, lead a normal lifestyle. At the end of the period, a second operation is performed to remove the correction plate.

Before the pandemic, similar experience exchange symposia for thoracic surgeons were held three times a year: in Kazan, St. Petersburg, and Krasnodar. Last year, the symposium was held only in Kazan, and the next two were canceled due to the coronavirus. Today, many thoracic surgery specialists met for the first time after a year and discussed their questions.

"One way or another, in all reports, we will return to the situation that exists today. We still do not know how to operate under covid conditions when operating on patients who have had a coronavirus. Yes, the infection exists, but people continue to suffer from other diseases and need to be treated. To date, these issues' solution relies on the experience and expert assessment of the surgeons themselves. However, soon, the game rules will be developed, " commented the leader of the thoracic service in Russia, Professor Vladimir Parshin.
